If you want to become a Distributor of Forever Living Products in Ukraine you should print and complete this form. After your sponsor puts his signature to it you should submit the form to the main FLPU office in Kiev where Company authorized representatives approve it.
Entry into the Company is free of charge.
If you (a Ukrainian Distributor) decide to sponsor yourself in a foreign country where there is an official office of the company "Forever Living Products", you should print and fill out this application form.
Submit the application to the main FLPU office in Kiev.
